Who, what and how to Report

This page provides detailed explanations on who has the obligation to report to the Agency, what is to be reported by these organisations and how it should be reported.
For those organisations for which the Agency is not the competent authority, there is no mandatory requirement to report to the Agency, but to their competent authority (see overview of legal framework).

Who shall report to the Agency?

  • All organisations for whom the Competent Authority is the Agency under the Regulation (EU) 2018/1139, its Implementing Rules and Regulation (EU) No 376/2014;
    Any organisation reporting to the Agency on voluntary basis.
     
  • Organisation-orientated-reporting
    • Holders of a type-certificate, restricted type-certificate, supplemental type-certificate, ETSO authorisation, major repair design approval or any other relevant approval issued or deemed to have been issued under Commission Regulation (EU) No 748/2012 (unless when bilateral agreement specify different provision)
    • Holders of a production organisation approval issued under Commission Regulation (EU) No 748/2012 (unless when bilateral agreement specify different provision)
    • Organisations approved under Commission Regulation (EU) No 1321/2014 Part 145, for whom the competent Authority is the Agency;
    • Organisations approved under Commission Regulation (EU) No 1321/2014 Part M, for whom the competent Authority is the Agency;
    • Pilot Training Organisation approved under Commission Regulation (EU) No 1178/2011 for whom the competent Authority is the Agency;
    • Air Navigation Service providers approved under Commission Implementing Regulation (EU) 2017/373 for whom the competent Authority is the Agency;

    What should be reported to the Agency?

    For holders of a type-certificate, restricted type-certificate, supplemental type-certificate, ETSO authorisation, major repair design approval or any other relevant approval issued or deemed to have been issued under Commission Regulation (EU) No 748/2012: occurrences listed in paragraph 2 of Annex II of the Commission Implementing Regulation (EU) 2015/1018.

    For holders of a production organisation approval issued under Commission Regulation (EU) No 748/2012, with its principal place of business outside a European Member State: occurrences according to Commission Regulation (EU) No 748/2012 Chapter 21.A.165(f)2, 21.A.3A and related AMCs and GMs (which include AMC 20-8).

    For holders of a production organisation approval issued under Commission Regulation (EU) No 748/2012, with its principal place of business inside a European Member State: occurrences listed in paragraph 1 of Annex II of the Commission Implementing Regulation (EU) No 2015/1018. 

    For organisations approved under Commission Regulation (EU) No 1321/2014 Part 145, for whom the competent Authority is the Agency: occurrences according to Commission Regulation (EU) No 1321/2014 Chapter 145.A.60 and related AMCs (which include AMC 20-8) and GMs.

    For organisations approved under Commission Regulation (EU) No 1321/2014 Part M, for whom the competent Authority is the Agency: occurrences according to Commission Regulation (EU) No 1321/2014 Chapter M.A.202 and related AMCs (which include AMC 20-8).

    For pilot training organisations approved under Commission Regulation(EU) No 1178/2011 for whom the competent Authority is the Agency: occurrences listed in Annex I and Annex V of the Commission Implementing Regulation (EU) No 2015/1018.

    For Air Navigation Service providers approved under Commission Implementing Regulation (EU) 2017/373 for whom the competent Authority is the Agency: occurrences listed in Annex III of the Commission Implementing Regulation (EU) No 2015/1018.

    Any organisation on a voluntary basis, any safety-related information deemed necessary to be brought to the awareness of the Agency.

    In addition, organisations should report to the Agency as their competent authority any event that has been assessed as representing a serious risk to the aircraft or its occupants.

    How to report to the Agency?

    In order to align the specific reporting requirements under the Implementing Rules (IRs) of Regulation (EU) 2018/1139 with the reporting requirements under the Regulation (EU) No 376/2014, two ways to report an occurrence or an unsafe condition to the Agency are possible:

    European Aviation Safety Reporting Portal

    The European Aviation Safety Reporting Portal provides common reporting forms for organisations and individuals to report to their competent authority, including the Agency, using a common and simple interface. This solution is meant for any individuals or organisations, in particular those organisations having a low number of occurrence reports per year. Please select the EASA Logo flag to make a report to the Agency.

    The European Aviation Reporting Portal (https://aviationreporting.eu) provides:  

    • On-line reporting via a web interface
    • Off-line reporting using a pdf form that may be downloaded from the portal and, once completed, shall be uploaded to the same portal.

    In both cases, the act of submitting the report will trigger a submission report to the competent authority.

    In the portal, there are five reporting forms (ATM/ANS, Flight Operations, Technical, Aerodrome and General Aviation). Organisations should choose the most adequate form for the type of occurrence which is being reported to the Agency (e.g., DOA, POA and EASA-145 would use the Technical form, while the ATO would use the Flight Operations form).

    E5X file format

    The E5X file format is the IT solution targeted for organisations to transfer relevant data from their Annex 19 database to an ECCAIRS environment of the competent authority in the Member State (MS) or the Agency. The E5X file format supersedes the previous IORS XSD v4 specifications that were utilised by high-volume occurrence reporting organisations, which EASA had acknowledged as Top Reporting Organisations (TROs). The E5X format is a compliant output file submitted directly by the organisation’s occurrence database to the competent authority.

    Any organisation that has the IT capability of generating an E5X compatible file could report their occurrence by uploading the E5X files to the Aviation reporting portal.

    The E5X files are considered valid if they adhere to the latest ECCAIRS taxonomy, or at least the previous version of the taxonomy. The most up-to-date version of the taxonomy can be found in the ECCAIRS 2 (E2) taxonomy browser.

    We strongly encourage EASA-approved organisations to complete the registration process by using the contact form and selecting the option "Request Access" (please provide your organisation name and approval number) and select EASA for the form to be sent to.

    Organisations seeking to establish direct and automatic communication between their system or application and ECCAIRS 2 (E2) and automatically submit occurrence reports to the agency can also utilise the E2 API (Application Programming Interface) which is a Machine2Machine method.

    These options are available only to registered organisations. To obtain additional details regarding the establishment of the connection between your application and the E2 API, kindly reach out to the ECCAIRS2 [at] easa.europa.eu (E2 support team).

    Organisations for which EASA is the competent authority under Regulation (EU) 2018/1139, its Implementing Rules, and Regulation (EU) No 376/2014 and wish to adopt this reporting format within their organisation are requested specifically to contact the SDM [at] easa.europa.eu (SDM Team). The EASA SDM Team will then provide the required technical details and verify the supplied E5X file. Organisations approved by an EU Member State should contact their respective competent authority(ies).

    Questionnaire on Suspected Unapproved Parts (SUP)

    When reporting an occurrence related to or concerning Suspected Unapproved Parts, please provide the SUP questionnaire as additional information. This questionnaire will facilitate the initial assessment of the reported issue and will enable a quicker reaction to address it. 

    Follow-up reports provision

    Please refer to the EASA guidance material on the provision of follow-up reports according to the Regulation (EU) No 376/2014 for Design and Production organisations.
