Information received during joint investigations conducted by the Federal Aviation Administration (FAA), the Department of Transportation-Office of Inspector General (DOT-IG) and the Defense Criminal Investigative Service (DCIS), revealed Mr. Jerry R. Comeaux d.b.a. Quality Aircraft Support located at 3200 Fruithill Road, Ninety Six, SC 29666, improperly returned auxiliary power units and component parts to service by producing documents similar to the FAA Form 8130-3, Airworthiness Approval Tag. The investigation also revealed Jerry R. Comeaux may have produced fraudulent FAA Form 8130-3, Airworthiness Approval Tag, representing the items as aircraft parts and certifying they were maintained in accordance with 14 Code of Federal Regulations (14 CFR) part 43. Jerry R. Comeaux formerly held FAA Air Agency Certificate No. QUYR410N from May 1996 to April 2006. Evidence revealed that Jerry R. Comeaux improperly returned parts to service while certificated as a repair station and continued approving parts to service after he had surrendered his air agency certificate on April 21, 2006. The FAA has been unable to determine all makes and models of affected aircraft and engines; therefore, all auxiliary power units and component parts approved for return to service by Jerry R. Comeaux d.b.a. Quality Aircraft Support should be considered suspect.
Discrepancies noted in Jerry Comeaux’s practices included, but are not limited to, the following:
• Approving parts for service using registered military cage code number 08HE3, as a certificate number on FAA Form 8130-3, Airworthiness Approval Tag.
• Failing to maintain recordkeeping requirements.
• Failing to ensure that all tools and test and inspection equipment used to make airworthiness determinations are calibrated to standards acceptable to the FAA.
• Approving for return to service articles that had not been maintained in accordance with the current manufacturer’s maintenance manual or methods otherwise acceptable to the administrator.
